Из фармакологических средств для лечения глаукомы различают препараты первой (наиболее эффективны и безопасны) и второй линии лечения. К первой линии лечения относятся аналоги простагландинов и бета-адреноблокаторы. Используемые в настоящее время аналоги простагландинов (латанопрост, биматопрост, тафлупрост и травопрост) являются аналогами PG F2α, осуществляющие своё действие через стимуляцию рецепторов FP. Их отличает оптимальное соотношение эффективности и риска побочного действия. Они удобны для пациента, т. к. для лечебного действия достаточно назначения 1 раз в день. Вследствие этого лечение глаукомы рационально начинать с одного из препаратов данной группы. С точки зрения фармакоэкономики наиболее доступным препаратом простагландинов является латанопрост, который в целом столь же эффективен, как и другие аналоги простагландинов. β-адреноблокаторы снижают продукцию ВГЖ, образование которой контролируется β1- и β2-адренорецепторами. Поэтому неселективные β-блокаторы (тимолол, левобунолол, метипранолол и картеолол) имеют фармакодинамическое преимущество перед селективными антагонистами β1-адренорецепторов (бетаксолол). Проведённые клинические исследования β-блокаторов показали, что, учитывая стоимость, эффективность и безопасность, тимолол оказался наиболее предпочтителен в терапии глаукомы. При наличии медицинских противопоказаний к применению препаратов первой линии или для усиления их эффективности используются α2-адреномиметики (апраклонидин и бримонидин), ингибиторы карбоангидразы (как правило, местного действия: дорзоламид и бринзоламид), М-холиномиметики (пилокарпин, карбахол и эхотиофат), а также ингибиторы Rho-киназы (рипасудил)</p></abstract><trans-abstract xml:lang="en"><p>Glaucoma is a disease associated with increased intraocular pressure (IOP). Of the pharmacological agents for treating glaucoma, there are drugs of the first (most effective and safe) and second-line treatment. First-line treatment includes prostaglandin analogs and beta-blockers. The currently used prostaglandin analogs (latanoprost, bimatoprost, tafluprost and travoprost) are PG F2α analogs that act through stimulation of FP receptors. They are distinguished by the optimal ratio of effectiveness and risk of side effects. They are convenient for the patient because for the therapeutic effect, it is enough to prescribe 1 time per day. As a result, it is rational to start the treatment of glaucoma with a drug in this group. In terms of pharmacoeconomics, the most affordable prostaglandin drug is latanoprost, which is generally as effective as other prostaglandin analogs. β-adrenergic blockers reduce the production of intraocular fluid, the formation of which is controlled by β1- and β2-adrenergic receptors. Therefore, non-selective β-blockers (timolol, levobunolol, metipranolol, and carteolol) have a pharmacodynamic advantage over selective β1-adrenergic antagonists (betaxolol). Conducted clinical studies of β-blockers have shown that given the cost, efficacy and safety, timolol was the most preferable treatment for glaucoma. Без лечения повышенное ВГД приводит к повреждению сетчатки и зрительного нерва с ограничением полей зрения и в конечном итоге к слепоте [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>]. Глобальная распространённость глаукомы среди населения в возрасте 40–80 лет составляет 3,54% [<xref ref-type="bibr" rid="cit3">3</xref>]. В Российской Федерации официальная заболеваемость глаукомой составляет 0,91 на 1000 взрослого населения, что практически вдвое меньше предполагаемых расчётных показателей. Распространённость первичной инвалидности вследствие глаукомы составляет 0,35 на 1000 взрослого населения, причём у 80,9% больных диагностируется открытоугольная глаукома [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>Различают два основных типа глаукомы: открытоугольная и закрытоугольная (или узкоугольная) [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>]. Закрытоугольная форма связана с неглубокой передней камерой, в которой расширенная радужка может перекрывать дренажный путь оттока под углом между роговицей и цилиарным телом [<xref ref-type="bibr" rid="cit1">1</xref>]. Эта форма связана с резким и болезненным повышением ВГД, которое необходимо в экстренном порядке контролировать с помощью лекарств или предотвращать хирургическим удалением части радужки (иридэктомия) [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>На долю открытоугольной глаукомы приходится от 72,3 до 96,1% всех форм глауком. Она является хроническим заболеванием, и лечение в основном носит фармакологический характер [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Ведущий симптом заболевания — повышение внутриглазного давления (ВГД). Внутриглазное давление при открытоугольной глаукоме повышается медленно и постепенно по мере нарастания сопротивления оттоку внутриглазной жидкости (ВГЖ). В начальный период повышение ВГД носит непостоянный характер, затем становится стойким.</p><p>Поскольку ВГД зависит от соотношения между двумя процессами — продукцией внутриглазной жидкости и её оттоком из глазного яблока, то направление терапии открытоугольной глаукомы заключается в двух направлениях: 1) уменьшение секреции внутриглазной жидкости; 2) усиление её оттока [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit5">5</xref>]. Ранняя диагностика и эффективное лечение способны замедлить прогрессирование заболевания и предотвратить необратимый распад зрительных функций [<xref ref-type="bibr" rid="cit4">4</xref>].</p><p>Из фармакологических средств для лечения глаукомы различают препараты первой (наиболее эффективны и безопасны) и второй линии лечения. К первой линии лечения относятся аналоги простагландинов и бета-адреноблокаторы.</p><p>Аналоги простагландинов. Простагландины — это небольшие молекулы, образующиеся в организме из арахидоновой кислоты, которые играют роль во множестве биологических процессов, включая воспаление, модуляцию боли, аллергию и формирование костей и многое другое [<xref ref-type="bibr" rid="cit13">13</xref>]. Используемые в настоящее время аналоги простагландинов (латанопрост, биматопрост, тафлупрост и травопрост) являются аналогами PG F2α, осуществляющие своё действие через стимуляцию рецепторов FP [<xref ref-type="bibr" rid="cit13">13</xref>]. В настоящее время продолжаются исследования и других рецепторов простагландинов с использованием таких соединений, как сулпростон (синтетический аналог простагландина E2, стимулирующий главным образом EP3-рецепторы) или илопрост (синтетический аналог простагландина IР2, стимулирующий IР-рецепторы) [<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>От прочих противоглаукомных средств аналоги простагландинов отличаются оптимальным соотношением эффективности (высокая) и риска побочного действия (низкий). Кроме того, они удобны для пациента, т. к. для лечебного действия достаточно назначения 1 раз в день [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit5">5</xref>][9–12]. Вследствие этого лечение глаукомы рационально начинать с одного из препаратов данной группы [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>С точки зрения фармакоэкономики и «кошелька» больного человека наиболее доступным препаратом простагландинов является латанопрост — пролекарство на основе сложного эфира простагландина (PG) F2α [<xref ref-type="bibr" rid="cit13">13</xref>][<xref ref-type="bibr" rid="cit14">14</xref>]. Он лучше всасывается, чем исходное соединение, через роговицу, а максимальная концентрация активного лекарственного средства находится в водянистой влаге через 1–2 часа после местного введения (15–30 нг/мл). Снижение ВГД, наблюдаемое при применении латанопроста, начинается через 3–4 часа, достигает максимума через 8–12 часов и сохраняется не менее 24 часов [<xref ref-type="bibr" rid="cit15">15</xref>]. Метаболизм препарата происходит в основном в печени.</p><p>Латанопрост (0,005%) был очень хорошо изучен в ходе клинических испытаний и метаанализов, которые показали, что он в целом столь же эффективен, как и другие аналоги простагландинов (биматопрост, травопрост и тафлупрост). Так, один из крупных мета-анализов включал 411 пациентов с открытоугольной глаукомой или глазной гипертензией, получавших в течение 12 недель латанопрост, биматопрост или травопрост [<xref ref-type="bibr" rid="cit16">16</xref>]. Среднее снижение ВГД составило 8,6 ± 0,3 мм рт. ст. с латанопростом, 8,7 ± 0,3 мм рт. ст. с биматопростом и 8,0 ± 0,3 мм рт. ст. при использовании травопроста. Между всеми тремя препаратами не наблюдалось значительных различий в снижении ВГД утром, в полдень, 16:00 и 20:00 или в изменениях средних суточных уровней ВГД. Исследование с участием 48 пациентов с открытоугольной глаукомой также не выявило статистически значимых различий между латанопростом и биматопростом в снижении ВГД в 8:00, 10:00, 13:00, 16:00, 20:00, 23:00 и 3:00 через 8 недель [<xref ref-type="bibr" rid="cit17">17</xref>].</p><p>Двойное слепое перекрёстное исследование, в котором определялись циркадные изменения ВГД у 44 пациентов с открытоугольной глаукомой или глазной гипертензией выявило, что через 1 месяц латанопрост, биматопрост и травопрост были одинаково эффективны в снижении ВГД без существенных различий между ними [<xref ref-type="bibr" rid="cit18">18</xref>]. В другом двойном слепом исследовании (64 пациента с открытоугольной глаукомой или глазной гипертензией) в любой временной точке, измеренной на 14-й или 29-й день тоже не было обнаружено статистически значимых различий в снижении ВГД между латанопростом (20–31%) и биматопростом (26–34%) [<xref ref-type="bibr" rid="cit19">19</xref>].</p><p>В перекрёстном исследовании оценивалось снижение ВГД у 54 пациентов с закрытоугольной глаукомой, получавших латанопрост или биматопрост в течение 6 недель. В конце лечения среднее значение ВГД снизилось на 8,4 ± 3,8 мм рт. ст., без существенных различий между группами [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Два более старых мета-анализа, каждый из которых включал более 1000 пациентов, также не выявили статистически значимых различий в эффективности снижения ВГД между латанопростом и биматопростом или травопростом [<xref ref-type="bibr" rid="cit34">34</xref>][<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>Применение травопроста, биматопроста и лантанопроста у 69 пациентов с глаукомой или глазной гипертензией способствовало уменьшению центральной толщины роговицы (p&lt;0,001) после среднего периода наблюдения в течение 17 месяцев [25–27]. Результаты лечения латанопростом (-14,95 ± 5,04 мкм), биматопростом (-17,00 ± 6,23 мкм) и травопростом (-15,73 ± 3,25 мкм) не отличались статистически [<xref ref-type="bibr" rid="cit27">27</xref>].</p><p>И, наконец, сравнение латанопроста с тафлупростом в рандомизированном двойном слепом исследовании фазы III, включавшем в себя 533 пациента с открытоугольной глаукомой или глазной гипертензией, тоже не выявило различий в их эффективности в снижении ВГД [<xref ref-type="bibr" rid="cit29">29</xref>]. Оба вещества существенно снизили ВГД: через 24 месяца после приёма латанопроста оно снизилось на 7,7 мм рт. ст., а при приёме тафлупроста — на 7,1 мм рт. ст. Эффект латанопроста был несколько больше, однако то, что тафлупрост не уступает ему в снижении суточного ВГД было показано с помощью дисперсионного анализа и с помощью анализа ковариации (верхние пределы 95% доверительных интервалов 1,38 и 1,52 соответственно). Предел не меньшей эффективности составлял 1,5 мм рт. ст. [<xref ref-type="bibr" rid="cit29">29</xref>]. Однако заслуживает внимания, что при этом 18 пациентов прекратили приём тафлупроста из-за его недостаточной эффективности, что значительно больше (3 пациента), чем в группе латанопроста [<xref ref-type="bibr" rid="cit29">29</xref>]. Ещё один мета-анализ, сравнивающий эффективность тафлупроста и латанопроста также дал заключение о сопоставимом действии на снижение ВГД при открытоугольной глаукоме и глазной гипертензии [<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Однако следует также отметить, что в некоторых исследованиях латанопрост уступал по эффективности иным производным простагландина. Так, исследование с участием 232 пациентов с глаукомой или глазной гипертензией показало, что биматопрост более значительно снижает ВГД, чем латанопрост в полдень (p=0,021), однако не в 8 часов утра (основной параметр эффективности), 16 или 20 часов после 3 месяцев лечения [<xref ref-type="bibr" rid="cit21">21</xref>]. В 7-недельном двойном слепом перекрёстном исследовании с участием 44 пациентов с открытоугольной глаукомой биматопрост был значительно эффективнее латанопроста в отношении суточной кривой ВГД только в 18:00 (p=0,008), но не в остальные сроки исследования [<xref ref-type="bibr" rid="cit22">22</xref>]. В трёхмесячном двойном слепом исследовании с участием 60 пациентов с глаукомой нормального давления среднее 24-часовое ВГД также было ниже (p=0,01) при приёме биматопроста (16,7 ± 2,4 мм рт. ст.), чем латанопроста (17,3 ± 2,8 мм рт. ст.). Значительные межгрупповые различия в среднем снижении ВГД в пользу биматопроста наблюдались в 8:00 (p≤0,033), но не в полдень или 16:00 [<xref ref-type="bibr" rid="cit23">23</xref>]. И, наконец, 6-месячное исследование с участием 269 пациентов с глаукомой или глазной гипертензией показало, что биматопрост более эффективен, чем латанопрост, в снижении ВГД во всех временных точках (p&lt;0,004) [<xref ref-type="bibr" rid="cit24">24</xref>]. Несмотря на это, в настоящее время сложился консенсус в том, что статистически значимых различий между различными представителями простагландинов нет [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Таким образом, большинство мета-анализов и клинических исследований, проведённых для оценки сравнительной эффективности различных производных простагландинов, показали, что они примерно равны по клинической эффективности в лечении глаукомы. В то же время латанопрост имеет хорошие краткосрочные и долгосрочные профили безопасности и переносимости [<xref ref-type="bibr" rid="cit14">14</xref>]. Как и другие простагландины, он не обладает системным действием, но может вызывать побочные эффекты со стороны глаз, такие как гиперемия конъюнктивы, пигментация радужной оболочки глаза, периокулярной кожи, гипертрихоз, а также раздражение глаз [<xref ref-type="bibr" rid="cit14">14</xref>]. Латанопрост переносится значительно лучше, чем биматопрост или травопрост [<xref ref-type="bibr" rid="cit16">16</xref>][<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit22">22</xref>][<xref ref-type="bibr" rid="cit24">24</xref>][<xref ref-type="bibr" rid="cit32">32</xref>][37–39]. Тафлупрост и латанопрост не отличались по частоте ощущения инородного тела, раздражения глаз, боли в глазах, гиперпигментации радужной оболочки, сухости глаз и зуда в глазах, однако тафлупрост хуже переносился пациентами из-за большей частоты гиперемии конъюнктивы [<xref ref-type="bibr" rid="cit36">36</xref>]. Пациенты, получавшие латанопрост, лучше соблюдают режим приёма и продолжают терапию дольше, чем пациенты, получающие другие препараты лечения глаукомы [40–42]. Недавно был разработан улучшенный состав латанопроста без консерванта бензалкония хлорида. Он так же эффективен, как и обычный латанопрост, имеет меньшую частоту возникновения гиперемии и может храниться при комнатной температуре [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>].</p><p>Таким образом, латанопрост, возможно, имеет наилучшее соотношение эффективности и переносимости среди аналогов простагландинов, доступных для лечения глаукомы, и поэтому обладает достаточно хорошей комплаентностью [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>β–адреноблокаторы. К препаратам первого ряда относятся также β–адреноблокаторы, которые занимают второе место по значению в лечении глаукомы [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Секреция ВГЖ эпителием цилиарного тела стимулируется ц-АМФ, которая продуцируется клетками радужки. β-адреноблокаторы, препятствуя активации G-белка, связанного с адренорецептором, нарушают возможность активировать аденилатциклазу, что снижает степень образования ц-АМФ [<xref ref-type="bibr" rid="cit5">5</xref>]. В структурах радужки продукция ВГЖ на 75–90% контролируется β2 -адренорецепторами и в значительно меньшей степени β1 -адренорецепторами.</p><p>Поскольку неселективные β-блокаторы (тимолол, левобунолол, метипранолол и картеолол) связываются с обоими подтипами рецепторов (β1 и β2), то они имеют фармакодинамическое преимущество перед селективными антагонистами β1 -адренорецепторов (бетаксолол). Однако у селективных антагонистов β1 -адренорецепторов имеется преимущество в безопасности приёма — они реже вызывают затруднение дыхания из-за блокады лёгочных β2-адренорецепторов [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Бета-адреноблокаторы в практике лечения глаукомы появились около 40 лет назад. Их терапевтическая эффективность при глаукоме и безопасность для больных была достаточно полно изучена в клинических исследованиях последних двух десятилетий ХХ века [45–47]. Основной вывод из данных исследований заключался в том, что тимолол, левобунолол, метипранолол и картеолол обладают сходной эффективностью в отношении снижения ВГД при глаукоме. Из них метипранолол наиболее доступен по стоимости лечения, но обладает наибольшей частотой побочных эффектов (жжение в глазах, покалывание и гранулематозный передний увеит). Контроль ВГД с помощью бетаксолола не всегда был таким хорошим, как с помощью тимолола; однако бетаксолол имеет некоторые преимущества перед тимололом и другими бета-адреноблокаторами для местного применения с точки зрения системных побочных эффектов. В то же время приём бетаксолол связан с более высокой частотой местных глазных побочных реакций, чем у тимолола [<xref ref-type="bibr" rid="cit47">47</xref>]. Проведённые исследования также показали, что учитывая стоимость, эффективность и безопасность бета-блокаторов, тимолол из них оказался наиболее предпочтителен в терапии глаукомы [<xref ref-type="bibr" rid="cit45">45</xref>][<xref ref-type="bibr" rid="cit46">46</xref>].</p><p>Большинство клинических исследований посвящены сравнительному изучению тимолола с аналогами простагландинов и ингибиторами карбангидразы.</p><p>Так, сравнивался 0,005% латанопрост (один раз в день) и 0,5% тимолола малеат (два раза в день) в качестве основной терапии у 30 пациентов с хронической первичной закрытоугольной глаукомой после лазерной иридотомии [<xref ref-type="bibr" rid="cit48">48</xref>]. Снижение ВГД было больше у пациентов, принимавших латанопрост (р&lt;0,001). В общей сложности 72% пациентов, принимавших латанопрост, и 43%, принимавших тимолол, добились снижения ВГД более чем на 30% по сравнению с исходным уровнем. Был сделан вывод, что достигнуто большее среднее и пиковое снижение ВГД при использовании 0,005% латанопроста один раз в день по сравнению с 0,5% тимолола два раза в день [<xref ref-type="bibr" rid="cit48">48</xref>]. Сходные результаты получены в рандомизированном двойном слепом многоцентровом 12-недельном исследовании, включавшем 275 пациентов с односторонней или двусторонней хронической закрытоугольной глаукомой (ХЗУГ), которые лечились латанопростом или тимололом [<xref ref-type="bibr" rid="cit49">49</xref>]. Средние изменения от исходного уровня суточного ВГД в течение 12 недель составили -8,2 мм рт. ст. и -5,2 мм рт. ст. для пациентов, получавших латанопрост и тимолол соответственно (разница: -3,0 мм рт. ст., р&lt;0,001). Оба препарата хорошо переносились [<xref ref-type="bibr" rid="cit49">49</xref>].</p><p>Эффективность и безопасность латанопроста по сравнению с тимололом изучалась также и у детей с глаукомой [<xref ref-type="bibr" rid="cit50">50</xref>]. В этом проспективном рандомизированном двойном слепом 12-недельном многоцентровом исследовании участвовало 137 пациентов (12–18 лет, n=48; 3–12 лет, n=55; младше 3 лет, n=34). Стратифицированные по возрасту, диагнозу и уровню ВГД дети были рандомизированы (1:1) на носитель латанопроста в 8 утра и латанопрост 0,005% в 8 вечера или тимолол 0,5% (0,25% для лиц в возрасте &lt;3 лет) два раза в день (8 утра, 8 вечера). Среднее снижение ВГД для латанопроста и тимолола на 12 неделе составило 7,2 и 5,7 мм рт. ст. соответственно, с разницей в 1,5 мм рт. ст. (95% ДИ, от -0,8 до 3,7; р=0,21). Частота достижения целевого результата составила 60% для латанопроста и 52% для тимолола (р=0,33). Таким образом, латанопрост 0,005% и тимолол обеспечивали клинически значимое снижение ВГД у педиатрических пациентов. При этом они имели благоприятные профили безопасности в течение этого 3-месячного испытания [<xref ref-type="bibr" rid="cit50">50</xref>].</p><p>В двойном слепом проспективном перекрёстном клиническом сравнительном исследовании 36 пациентам с первичной открытоугольной глаукомой с неконтролируемым ВГД, несмотря на лечение латанопростом, применяемым один раз в день, дважды в день вводили тимолол и дорзоламид [<xref ref-type="bibr" rid="cit51">51</xref>]. Последовательность лечения была рандомизирована. При добавлении к латанопросту и дорзоламид, и тимолол значительно снижали ВГД на циркадной кривой (р&lt;0,05). Дорзоламид снижал исходные значения ВГД в каждый момент времени. Тимолол также снижал исходные значения ВГД во все моменты времени, кроме 03:00. Целевые показатели снижения составили 86% для группы дорзоламида и 61% для группы тимолола. Был сделан вывод, что обе комбинации эффективны в снижении ВГД, при этом процент снижения выше при режиме латанопрост плюс дорзоламид, особенно в ночное время [<xref ref-type="bibr" rid="cit51">51</xref>].</p><p>В мета-анализе, где сравнивалась эффективность монотерапии тимололом, латанопростом, биматопростом или травопростом при открытоугольной глаукоме были включены девять рандомизированных клинических испытаний с участием 1090 пациентов [<xref ref-type="bibr" rid="cit30">30</xref>]. Было показано, что разница в абсолютном снижении ВГД между аналогами простагландина и тимололом варьировала от 0,4 до 1,6 мм рт. ст., а по суточной кривой — от 0,9 до 2,3 мм. рт. ст. [<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>С помощью мета-регрессии был проведён анализ изменения ВГД с течением времени у 2943 пациентов (18 исследований) с первичной открытоугольной глаукомой [<xref ref-type="bibr" rid="cit32">32</xref>]. Относительные эффекты лечения оценивались с использованием сравнения смешанного лечения со случайными эффектами, чтобы сохранить рандомизацию и избежать смещения выбора. Для получения клинически значимых результатов эти анализы были объединены для получения среднего значения ВГД на фоне лечения и доли пациентов, достигших различных целевых показателей ВГД в разные моменты времени. Кроме того, оценена вероятность получить глазную гиперемию [<xref ref-type="bibr" rid="cit52">52</xref>]. Анализ показал, что после 3 месяцев лечения у от 58 до 83% пациентов ВГД снижалось не менее чем на 20%; 70–93% пациентов имели абсолютное ВГД менее 20 мм рт. ст. При этом латанопрост и биматопрост, обладая примерно одинаковой эффективностью, обеспечивали значительно более низкое ВГД по сравнению с тимололом (р&lt;0,05). Травопрост, хотя и обеспечивал более низкое среднее ВГД по сравнению с тимололом, но данное различие было незначительным. Комбинация латанопрост-тимолол обеспечивала более низкое ВГД, чем только латанопрост или только бета-блокаторы [<xref ref-type="bibr" rid="cit32">32</xref>]. Эффективность 24-часового снижения ВГД у 386 пациентов показала статистически значимую эффективность монотерапии аналогами простагландинов, по сравнению с тимололом, бримонидином, дорзоламидом [<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>При наличии медицинских противопоказаний к применению препаратов первой линии — аналогов простагландинов или β-адреноблокаторов — в качестве их заменителей используются α2 -адреномиметики, ингибиторы карбоангидразы (как правило, местного действия), М-холиномиметики, а также ингибиторы Rho-киназы [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Альфа2-адреномиметики обеспечивают снижение ВГД за счёт уменьшения образования внутриглазной жидкости и усиления оттока жидкости из передней камеры глаза как из-за прямой стимуляции α2-адренорецепторов, так и косвенно за счёт повышения синтеза простагландинов [<xref ref-type="bibr" rid="cit5">5</xref>]. Основными препаратами группы являются апраклонидин и бримонидин.</p><p>Апраклонидин схож по фармакологическим свойствам с клонидином, однако он не проникает через гематоэнцефалический барьер и поэтому практически лишён центральных эффектов (угнетение, обезболивание) и не влияет на уровень артериального давления [<xref ref-type="bibr" rid="cit5">5</xref>]. Бримонидин — это селективный агонист α2 -адренорецепторов, липофильные свойства которого позволяют легко проникать через роговицу. Оба препарата воздействуют как на пресинаптические, так и на постсинаптические α2 –адренорецепторы [<xref ref-type="bibr" rid="cit5">5</xref>]. Связываясь с пресинаптическими рецепторами, они уменьшают в симпатических нервах освобождение норадреналина в синаптическую щель, а за счёт связывания с постсинаптическими α2 -адренорецепторами снижают образование вторичного клеточного мессенжера цАМФ. Всё это способствует угнетению образования внутриглазной жидкости [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Степень снижения ВГД при применении апраклонидина в качестве монотерапии аналогична эффективности тимолола малеата 0,5% в течение восьми часов после введения, но меньше, чем у тимолола малеата через 12 часов после инстилляции. В качестве дополнительного вещества в комбинированном лечении глаукомы апраклонидин снижает внутриглазное давление примерно на 16% по сравнению с монотерапией малеатом тимолола. Эти клинические эффекты стабильны в течение трёх месяцев после начала терапии [<xref ref-type="bibr" rid="cit52">52</xref>]. Апраклонидин также полезен для предотвращения временного повышения внутриглазного давления после лазерных процедур переднего сегмента глаза (т. е. на роговице, радужке и хрусталике) и внутриглазных операций. Он хорошо переносится без каких-либо серьёзных побочных эффектов; могут появиться симптомы зуда, а также жжения и признаки воспаления конъюнктивы, что быстро проходят после отмены лекарства [<xref ref-type="bibr" rid="cit52">52</xref>].</p><p>Агонист α2 -адренергических рецепторов бримонидин достаточно эффективно снижает ВГД и используется в качестве монотерапии, дополнительной терапии и заместительной терапии при открытоугольной глаукоме и глазной гипертензии [<xref ref-type="bibr" rid="cit53">53</xref>].</p><p>Исследования [54–57] показали, что 0,2% бримонидин, применяемый дважды в день при 12-месячном назначении, снижал ВГД так же, как тимолол на пиковом эффекте (через 2 часа после приёма), но уступал ему при назначении утром. Эффективность бримонидина сохранялась при длительном применении, и после четырёх лет лечения бримонидин и тимолол обеспечивали сопоставимое снижение ВГД как на пиковом, так и на минимальном уровне [<xref ref-type="bibr" rid="cit58">58</xref>]. Фиксированная комбинация бримонидина и тимолола снижает ВГД в такой же степени, как комбинированная терапия по отдельности бримонидином и тимололом, но более удобна для пациента [<xref ref-type="bibr" rid="cit60">60</xref>].</p><p>Дополнительным преимуществом бримонидина при лечении глаукомы и глазной гипертензии является его нейропротективная активность: он защищает ганглиозные клетки сетчатки от повреждения и гибели независимо от его воздействия на ВГД [<xref ref-type="bibr" rid="cit53">53</xref>][<xref ref-type="bibr" rid="cit61">61</xref>][<xref ref-type="bibr" rid="cit62">62</xref>].</p><p>В настоящее время нашла применение композиция бримонидина — бримонидин-Пурит 0,1%. Она имеет более высокий pH, что улучшает биодоступность бримонидина при местном применении, поэтому бримонидин-Пурит 0,1% равно эффективен 0,2% раствору бримонидина [<xref ref-type="bibr" rid="cit53">53</xref>].</p><p>Бримонидин безопасен и хорошо переносится, т. к., попадая в системный кровоток после местного применения, быстро метаболизируется и имеет короткий (около 2 ч) период полувыведения из плазмы [<xref ref-type="bibr" rid="cit53">53</xref>][<xref ref-type="bibr" rid="cit55">55</xref>]. Так, в отличие от антагонистов β-адренорецепторов, его применение не имеет кардиопульмонологических противопоказаний, и бримонидин может безопасно и эффективно использоваться пациентами, получающими системную антигипертензивную терапию β-адреноблокаторами [<xref ref-type="bibr" rid="cit53">53</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. В отличие от аналогов простагландинов, бримонидин не вызывает рост ресниц или пигментацию радужной оболочки или век [<xref ref-type="bibr" rid="cit53">53</xref>]. Побочные эффекты, связанные с лечением бримонидином, обычно носят местный характер и включают гиперемию конъюнктивы, аллергический конъюнктивит и зуд глаз [<xref ref-type="bibr" rid="cit53">53</xref>]. Наиболее частыми системными побочными эффектами являются сухость во рту, утомляемость или сонливость [<xref ref-type="bibr" rid="cit53">53</xref>]. При назначении бримонидина в течение 1 года риск аллергии составляет 11,5% [<xref ref-type="bibr" rid="cit57">57</xref>].</p><p>Ингибиторы карбоангидразы (ИКА) снижают ВГД за счёт угнетения активности фермента карбоангидразы в отростках цилиарного тела, что уменьшает секрецию внутриглазной жидкости [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit63">63</xref>]. ИКА для лечения глаукомы подразделяются на два поколения: системные ИКА (или ИКА I поколения) и препараты для местного применения (ИКА II поколения), которые представлены дорзоламидом (2% глазные капли, а также комбинированным препаратом с 0,5% тимололом) и бринзоламидом (1% глазные капли (суспензия), а также его комбинированный препарат с 0,2% бримонидином) [<xref ref-type="bibr" rid="cit5">5</xref>]. Дорзоламид и бринзоламид в целом равно эффективны и снижают ВГД на 15–20%, а системные ИКА ацетазоламид и метазоламид (применяются в виде таблеток) — на 20–30% [9 64, 65]. Ацетазоламид применяют также и при остром приступе глаукомы [<xref ref-type="bibr" rid="cit5">5</xref>]. Однако несистемные ИКА более популярны из-за лучшей переносимости пациентами [5, 9].</p><p>Нередкие побочные эффекты системных ингибиторов карбоангидразы включают в себя парестезии (стопы, кисти), дискомфорт в желудке, гипокалиемия, почечные камни, аллергические реакции. У ацетазоламида дискомфорт в желудке и парестезии встречаются чаще, чем у метазоламида [<xref ref-type="bibr" rid="cit5">5</xref>]. У него же зарегистрированы и очень редкие, но иногда тяжёлые, побочные эффекты: острая почечная недостаточность, паралитический илеус, тромбоцитопения, близорукость в высокогорье, синдром Стивена-Джонсона [66–70]. Вследствие этого ИКА I поколения относят к препаратам резерва [<xref ref-type="bibr" rid="cit5">5</xref>]. Напротив, ИКА II поколения вызывают побочные эффекты сравнительно редко — при применении дорзоламида (чаще) и бринзоламида (реже) возможны жжение, и покалывание в глазу, и системный побочный эффект — металлический вкус во рту [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>К настоящему времени сложилась практика терапевтического применения ингибиторов карбоангидразы II поколения для повышения эффективности препаратов простагландинов или бета-адреноблокаторов. На основе ряда мета-анализов и отдельных клинических исследований был сделан вывод, что дорзоламид или бринзоламид в качестве дополнения к простагландинам или бета-адреноблокаторам эффективно снижали ВГД у пациентов с рефрактерной глаукомой или глазной гипертензией, не вызывая при этом существенных побочных реакций [71–73].</p><p>Холинергические агонисты (миотики). Возбуждение М3 -холинорецепторов круговой мышцы радужки приводит к её сокращение и сужению зрачка, что влечёт за собой открытие шлеммова канала и повышение оттока внутриглазной жидкости. В итоге ВГД снижается. Кроме того, возбуждение М3 -холинорецепторов цилиарной мышцы приводит к её сокращению, что также увеличивает отток из шлеммова канала [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit74">74</xref>]. Из препаратов этого класса в настоящее время наиболее известны пилокарпин, карбахол и эхотиофат [<xref ref-type="bibr" rid="cit75">75</xref>]. Клинические доказательства эффективности холинергических агонистов ограничены. Так, оценка эффективности ацетилхолина и карбахола при хирургии катаракты проведена всего в двух сравнительных клинических испытаниях, опубликованных в 1971 и 1972 годах [75–77]. Рандомизированное контролируемое исследование 18 пациентов с глазной гипертензией, получавших 2% пилокарпин в один глаз и физиологический раствор в другой глаз три раза с 10-минутными интервалами показало значительное снижение ВГД и значительное увеличение пульсирующего глазного кровотока [<xref ref-type="bibr" rid="cit78">78</xref>]. О побочных эффектах не сообщалось. Согласно этому исследованию 2% пилокарпин может быть эффективным вариантом лечения глазной гипертензии [<xref ref-type="bibr" rid="cit78">78</xref>]. Кроме того, в рандомизированном контролируемом исследовании у 30 пациентов, получавших низкие дозы внутрикамерного пилокарпина в стерильном ирригационном растворе (0,13 мг/мл) в один глаз и стерильный промывочный раствор в другой глаз в конце процедуры факоэмульсификации для удаления вязкоупругих веществ и стимулирования сужения зрачка, было показано, что внутрикамерный пилокарпин может быть эффективен для сужения зрачка после такой процедуры без отрицательного воздействия на эндотелий роговицы [<xref ref-type="bibr" rid="cit79">79</xref>].</p><p>В настоящее время холинергические средства используются нечасто из-за потенциальных побочных эффектов и неудобного режима дозирования [<xref ref-type="bibr" rid="cit5">5</xref>]. Побочные эффекты холинергических средств, чаще только местные, включают миопию, боль в глазах, спазм аккомодации и раздражение глаз [<xref ref-type="bibr" rid="cit76">76</xref>][<xref ref-type="bibr" rid="cit80">80</xref>][<xref ref-type="bibr" rid="cit81">81</xref>]. Системные нежелательные реакции наблюдались редко и включали в себя тошноту, рвоту, брадикардию, диарею и повышенное слюноотделение и потоотделение [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit82">82</xref>]. Побочные эффекты при терапии карбахолом, как правило, тяжелее и чаще, чем при назначении пилокарпина [<xref ref-type="bibr" rid="cit84">84</xref>]. Кроме того, карбахол плохо всасывается через роговицу и обычно назначается только при неэффективности пилокарпина [<xref ref-type="bibr" rid="cit85">85</xref>]. Эхотиофат используется редко и только при неэффективности других методов лечения глаукомы, т. к. его применение связано с повышенным риском системной токсичности и серьёзными побочными эффектах со стороны глаз (образование катаракты, отслоение сетчатки и образование кисты радужки) [<xref ref-type="bibr" rid="cit71">71</xref>][<xref ref-type="bibr" rid="cit85">85</xref>].</p><p>Ингибиторы Rho-киназы. Рипасудил — единственный ингибитор Rho-киназы, имеющийся в настоящее время на фармацевтическом рынке, для лечения глаукомы. Rho-киназа — это серин / треониновая киназа с киназным доменом, который участвует в регуляции сокращения гладких мышц и образования стрессовых волокон [<xref ref-type="bibr" rid="cit86">86</xref>]. Были идентифицированы две изоформы Rho-киназы: ROCK-I (ROCK β) и ROCK-II (ROCK α). ROCK II играет патофизиологическую роль при глаукоме, и, следовательно, ингибиторы ROCK могут быть полезными для уменьшения потери зрения [<xref ref-type="bibr" rid="cit86">86</xref>]. Эти ингибиторы снижают внутриглазное давление в глаукомном глазу за счёт увеличения оттока водянистой влаги через трабекулярную сеть. Они также действуют как средства против рубцевания и, следовательно, предотвращают послеоперационное рубцевание [<xref ref-type="bibr" rid="cit86">86</xref>]. Они также обладают нейропротективной активностью, механизм которой заключается в регенерации аксонов за счёт увеличения кровотока в зрительном нерве, что может быть полезно при лечении повреждений волокон зрительного нерва. Ингибиторы ROCK защищают нейроны центрального зрительного пути, прерывая апоптоз RGC, что является новым фармакологическим подходом к нейропротекции при глаукоме [<xref ref-type="bibr" rid="cit86">86</xref>].</p><p>Эффективность и безопасность рипасудила были доказаны в проспективном открытом обсервационном исследовании у 3359 пациентов с первичной открытоугольной глаукомой (43,9%), глаукомой нормального давления (36,6%), вторичной глаукомой (8,7%), внутриглазной гипертензией (4,2%) и первичной закрытоугольной глаукомой (2,4%). Приём рипасудила на фоне стандартного лечения [<xref ref-type="bibr" rid="cit84">84</xref>]. Значительное снижение ВГД через 12 месяцев в среднем до 18,1 ± 6,1 мм рт. ст. (p&lt;0,001) наблюдалось у пациентов при всех типах глаукомы за исключением неоваскулярной глаукомы. Значимое снижение ВГД происходило независимо от того, назначался ли он в качестве монотерапии или в комбинации с другими противоглаукомными средствами, и сохранялось в течение 12 месяцев после терапии. Нежелательные реакции наблюдались у 18,6% пациентов, наиболее частыми были гиперемия конъюнктивы и блефарит. Нежелательные реакции, связанные с аллергией и/ или воспалением, наблюдались у 11,6% пациентов, чаще всего при блефарите (5,6%) и конъюнктивите (4,2%) [<xref ref-type="bibr" rid="cit87">87</xref>].</p><p>Таким образом, рипасудил оказался безопасным и эффективным у пациентов с глаукомой или внутриглазной гипертонией на фоне их обычного лечения.</p><p>В заключение можно сказать, что на сегодняшний день лекарственное воздействие на глаз является доминирующим методом контроля внутриглазного давления у пациентов с открытоугольной формой глаукомы. Правильное применение офтальмогипотензивных средств позволяет осуществлять качественный контроль над течением заболевания, что препятствует или замедляет развитие необратимых процессов в структурах глаза, способствуя, таким образом, длительному сохранению зрительных функций у больного человека.</p></sec><sec><title>ДОПОЛНИТЕЛЬНАЯ ИНФОРМАЦИЯ / ADDITIONAL INFORMATION</title><p>Конфликт интересов.
