The use of bromhexine is possible in clinical trials.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>коронавирус</kwd><kwd>бромгексин</kwd><kwd>COVID-19</kwd></kwd-group><kwd-group xml:lang="en"><kwd>coronavirus</kwd><kwd>bromhexine</kwd><kwd>COVID-19</kwd></kwd-group></article-meta></front><body><sec><title>Введение</title><p>Бромгексин — муколитическое лекарственное средство, не зарегистрированное на территории США, но применяющееся в Германии и России. В организме человека бромгексин превращается в амброксол путём гидроксилирования. Амброксол и бромгексин имеют сходные пути метаболизма [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>]. В 2015 году опубликовано резюме исследований, посвящённых поиску ингибиторов сериновой трансмембранной протеазы (TMPRSS2), которая принимает участие во внедрении опухолевых клеток рака предстательной железы [<xref ref-type="bibr" rid="cit3">3</xref>]. Эта же протеаза, согласно появляющимся данным, экспрессируется в эпителии лёгких человека и принимает участие в процессе внедрения различных вирусов (включая вирусы гриппа и коронавирусы, в т. ч. SARS-CoV2) [<xref ref-type="bibr" rid="cit4">4</xref>]. В этой связи появилось предположение о возможности применения бромгексина для профилактики инфицирования SARS-CoV-2 медицинскими работниками [<xref ref-type="bibr" rid="cit5">5</xref>], что должно иметь преимущества перед другими способами профилактики (в частности, гидроксихлорохином), учитывая его предполагаемую существенно более низкую токсичность.</p></sec><sec><title>Возможность применения при COVID-19</title><p>В упомянутой серии экспериментов авторы скринировали библиотеки химических компонентов и выделили бромгексин как вещество, потенциально способное ингибировать TMPRSS2. В серии экспериментов на мышах с моделированным раком простаты бромгексин в дозе, составляющей 2 % от LD50, достоверно уменьшал формирование метастазов. В экспериментах с рекомбинантным TMPRSS2 бромгексина гидрохлорид угнетал активность протеазы, однако не полностью [<xref ref-type="bibr" rid="cit3">3</xref>]. На официальном сайте международного фармакологического общества [<xref ref-type="bibr" rid="cit6">6</xref>] в качестве субстрата этой протеазы упомянуты зарегистрированные в Японии средства камостата мезилат, использующийся для лечения панкреатита), и нафамостат, которые продемонстрировали ингибирующую активность в исследованиях [<xref ref-type="bibr" rid="cit7">7</xref>]. В исследовании in vitro было показано, что камостат ингибирует вхождение частиц вирусов SARS-CoV-2 в клетку [<xref ref-type="bibr" rid="cit8">8</xref>]. В исследовании, опубликованном индийскими учёными [<xref ref-type="bibr" rid="cit9">9</xref>], были подтверждены сведения о том, что структура бромгексина такова, что может ингибировать TMPRSS2, но также и то, что ингибирующая его способность существенно ниже, чем у камостата и нафамостата.</p></sec><sec><title>Наличие клинических исследований, клинических руководств, протоколов ведения пациентов с COVID-19</title><p>На сайте <ext-link xlink:href="http://www.ClinicalTrials.gov" ext-link-type="uri">www.ClinicalTrials.gov</ext-link> на 01.05.2020 г. зарегистрировано 3 исследования, локализованные в Словении, Мексике, Китае, в которых планируется исследовать бромгексин как дополнение к умифеновиру или интерферону альфа в лечении лёгких форм COVID-19, как дополнение к гидроксихлорохину для профилактики COVID-19 у медицинских работников и как дополнение к гидроксихлорохину в лечении пациентов с COVID-19 ассоциированной с пневмонией. Других данных о применении бромгексина у человека для лечения или профилактики COVID-19 в доступной научной литературе не найдено.</p></sec><sec><title>Теоретический анализ возможности достижения эффективных концентраций у человека</title><p>При применении у человека обращает внимание то, что бромгексин характеризуется низкой биодоступностью: только 25 % достигают системного кровотока, а 75 % подвергаются метаболизму при первичном прохождении через печень [<xref ref-type="bibr" rid="cit1">1</xref>]. В исследовании Lucas JM et al., где бромгексин продемонстрировал эффекты у мышей, применяли дозу 30 мг/кг, что составляет 2,4 мг/кг или 183 мг для человека массой 75 кг [<xref ref-type="bibr" rid="cit3">3</xref>]. Максимальная допустимая доза для человека, согласно зарегистрированным инструкциям, составляет 64 мг [<xref ref-type="bibr" rid="cit10">10</xref>]. Учитывая интраперитонеальное введение препарата в эксперименте, эквивалентная доза для человека при пероральном приёме, при 75 % первичном метаболизме, составит 320 мг. Дозы бромгексина, подавлявшие активность TMPRSS2 в эксперименте, варьировали в интервале 0—25 мкМ.</p><p>Исследования фармакокинетики бромгексина у человека демонстрировали максимальные концентрации в плазме около 27—377 нМ/л после приёма таблетки 8 мг [<xref ref-type="bibr" rid="cit11">11</xref>] — в тысячу раз меньше, что ставит под сомнение возможность практического применения препарата.</p></sec><sec><title>Заключение</title><p>Материал подготовлен: 01.05.2020 г.</p></sec></body><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Ishiguro N, Senda C, Kishimoto W, Sakai K, Funae Y, Igarashi T.
