Подагра является распространённой воспалительной артропатией, характеризующейся гиперурикемией, окислительным стрессом и эндотелиальной дисфункцией.</p></sec><sec><title>Цель</title><p>Цель. Изучить взаимосвязь между эндотоксин-связывающими системами и  концентрацией эндотелиальной NO-синтазы (NOS3) у пациентов с подагрой. Материалы и методы. В исследование включены 41 пациент с подтверждённым диагнозом подагра согласно критериям классификации EULAR/ACR и 33 здоровых человека, формирующих контрольную группу. В плазме измеряли концентрации: вчСРБ (высокочувствительный С-реактивный белок), NOS3 (nitric oxide synthase 3; эндотелиальная синтаза оксида азота 3) и BPI (Bactericidal/Permeability-Increasing Protein) методом твердофазного иммуноферментного анализа (ИФА). Для оценки связи между уровнями BPI и NOS3 применяли линейную регрессию и рассчитали коэффициент корреляции Пирсона (r), коэффициент детерминации (R²) и уровень значимости (p). Использовалось программное обеспечение IBM SPSS Statistics 26.0.</p></sec><sec><title>Результаты</title><p>Результаты. У  пациентов с  подагрой выявлены повышенные уровни липополисахарид-связывающего белка (ЛСБ) и высокочувствительного С-реактивного белка, а также значительно сниженная концентрация BPI по сравнению с контрольной группой (p&lt;0,001). Регрессионный анализ показал статистически значимую положительную корреляцию между уровнями BPI и NOS3 (r = 0,605, p&lt;0,001), при этом 36,6 % дисперсии NOS3 объясняется изменениями концентрации BPI.</p></sec><sec><title>Выводы</title><p>Выводы. Полученные данные свидетельствуют о тесной взаимосвязи между системами врождённого иммунитета и эндотелиальной функцией при подагре. BPI может обладать протективным действием, поддерживающим функцию эндотелия в условиях хронического воспаления и окислительного стресса. В дальнейшем возможна разработка комплексной биомаркерной панели BPI+NOS3 для мониторинга или прогноза сосудистых осложнений и индивидуализации терапии при подагре.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Gout is a common inflammatory arthropathy characterized by hyperuricemia, oxidative stress, and endothelial dysfunction.</p></sec><sec><title>Objective</title><p>Objective: To study the relationship between endotoxin-binding systems and the concentration of endothelial NO synthase (NOS3) in patients with gout.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 41 patients with a confirmed diagnosis of gout according to the EULAR/ACR classification criteria and 33 healthy people forming the control group. Plasma concentrations were measured: highly sensitive C-reactive protein (hsCRP) NOS3 (eNOS), BPI by solid-phase ELISA. Linear regression was used to assess the relationship between BPI and NOS3 levels and the Pearson correlation coefficient (r), coefficient of determination (R2), and signifi cance level (p) were calculated. IBM SPSS Statistics 26.0 soft ware was used.</p></sec><sec><title>Results</title><p>Results. Patients with gout showed elevated levels of lipopolysaccharide-binding protein (LBP) and highly sensitive C-reactive protein, as well as significantly reduced BPI concentrations compared with the control group (p&lt;0.001). Regression analysis showed a statistically significant positive correlation between BPI and NOS3 levels (r = 0.605, p&lt;0.001), with 36.6 % of the NOS3 variance attributed to changes in BPI concentration.</p></sec><sec><title>Conclusion</title><p>Conclusion. The data obtained indicate a close relationship between innate immune systems and endothelial function in gout. BPI may have a protective effect that supports endothelial function in conditions of chronic inflammation and oxidative stress.
