Хроническая сердечная недостаточность (ХСН) — клинический синдром, являющийся широко распространённым осложнением сердечно-сосудистых заболеваний. Известно, что большую часть пациентов с ХСН составляют лица с сохранённой фракцией выброса левого желудочка (ФВ ЛЖ ≥ 50 %) (ХСНсФВ). Количество таких больных увеличивается с каждым годом, при этом эффективной медикаментозной терапии данного заболевания не существует.</p></sec><sec><title>   Цель</title><p>   Цель. Составить портрет пациента, имеющего ХСНсФВ, и проанализировать показатели 10-летней выживаемости по шкале Charlson Comorbidity Index в когорте пациентов с учётом гендерного и возрастного признаков.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. В исследовании приняли участие пациенты с ХСНсФВ. Учитывались демографические, анамнестические, лабораторные инструментальные данные. Оценка 10-летней выживаемости проводилась по шкале Charlson Comorbidity Index.</p></sec><sec><title>   Результаты</title><p>   Результаты. Средний возраст исследуемых составил 62 ± 26 лет. Среди патологий сердечно-сосудистой системы преобладали гипертоническая болезнь — 274 чел. (100 %), стенокардия — 184 чел. (67 %), фибрилляция предсердий (ФП) — 100 чел. (36 %). Среди исследуемых у 14 чел. (5 % пациентов) в анамнезе имелась хроническая обструктивная болезнь лёгких (ХОБЛ), у 8 чел. (3 %) — бронхиальная астма (БА). 88 исследуемых (33 %) перенесли сердечно-сосудистую катастрофу. Среди всех исследуемых показатель 10-летней выживаемости по шкале Charlson Comorbidity Index менее 50 % выявлен у 174 человек (63,5 %), среди которых преобладают лица женского пола — 106 человек (39 %).</p></sec><sec><title>   Заключение</title><p>   Заключение. Портрет типичного пациента с ХСНсФВ: пациент мужского или женского пола в возрасте от 60 до 69 лет. Из сопутствующей патологии чаще всего имеющий избыточную массу тела/ожирение, гипертоническую болезнь, стенокардию, атеросклероз, ФП, сахарный диабет, перенёсший в анамнезе инфаркт миокарда и/или инсульт/транзиторную ишемическую атаку. Индекс выживаемости менее 50 % по шкале Charlson Comorbidity Index чаще имеют женщины пожилого возраста (от 60 до 74 лет).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. Chronic heart failure (CHF) is a clinical syndrome that is a widespread complication of cardiovascular diseases. The majority of patients with CHF are individuals with a preserved left ventricular ejection fraction (LVEF ≥ 50 %). The number of such patients is increasing every year, while there is no effective drug therapy for this disease.</p></sec><sec><title>   Objective</title><p>   Objective. To create a portrait of a patient with CHF and analyze the 10-year survival rates on the Charlson Comorbidity Index scale in a cohort of patients, considering gender and age characteristics.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. Patients with CHF participated in the study. Demographic, anamnestic, and laboratory instrumental data were considered. The 10-year survival rate was assessed using the Charlson Comorbidity Index scale.</p></sec><sec><title>   Results</title><p>   Results. The average age of the subjects was 62 ± 26 years. Among the pathologies of the cardiovascular system, hypertension prevailed — 274 people (100 %), angina pectoris — 184 people (67 %), and atrial fibrillation (AF) — 100 people (36 %). Among the studied 14 people. (5 % of patients) had a history of chronic obstructive pulmonary disease (COPD), 8 people (3 %) had bronchial asthma (BA). 88 subjects (33 %) suffered a cardiovascular catastrophe. Among all the studies, the 10-year survival rate on the Charlson Comorbidity Index scale of less than 50 % was detected in 174 people (63.5 %), among whom 106 women predominated (39 %).</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Portrait of a typical patient with CHF: male or female patient aged 60–69 years. Of the concomitant pathologies, most often those who are overweight / obese, hypertension, angina pectoris, atherosclerosis, AF, diabetes mellitus, who have had a history of myocardial infarction and/or stroke/transient ischemic attack.
