В настоящее время фторхинолоны продолжают широко применяться в лечении большого круга инфекционных заболеваний, однако в процессе их использования выявлены новые, в том числе серьёзные риски. Для сохранения благоприятного соотношения «польза-риск» необходимо продолжить тщательное мониторирование профиля безопасности фторхинолонов в реальной клинической практике, что позволит, в том числе разработать новые подходы к их рациональному назначению.</p></sec><sec><title>   Цель</title><p>   Цель. Изучить структуру нежелательных реакций, ассоциированных с применением фторхинолонов, на основе ретроспективного анализа национальной базы данных спонтанных сообщений.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Проведён ретроспективный анализ спонтанных сообщений (СС), поступивших в российскую базу данных о нежелательных реакциях (НР) в период с 01. 04. 2019 по 28. 02. 2023 гг. Критерии включения в исследование: СС, по информации которых НР произошли на территории Российской Федерации, в качестве подозреваемого лекарственного препарата (ЛП) указывался антибиотик из группы фторхинолонов и была установлена высокая причинно-следственная связь «НР-ЛП». Для кодирования и систематизации НР использовалась русскоязычная версия MedDRA.</p></sec><sec><title>   Результаты</title><p>   Результаты. В исследование было включено 1890 СС, на левофлоксацин пришлось 950 СС, на ципрофлоксацин — 633, на моксифлоксацин — 205, на остальные фторхинолоны — 102. 75,5 % поступивших сообщений содержали информацию о серьёзных НР, более 80 % НР развились на стационарном этапе оказания медицинской помощи, в 2,9 % случаев выявлено самолечение. 40,8 % СС о НР при применении фторхинолонов приходилось на молодой возраст; возраст пациентов ≥ 75 лет указывался в 7,7 % СС; в 75 случаях (4,0 %) фторхинолоны назначались детям до 18 лет. Проведённый анализ показал, что чаще всего сообщались НР, относящиеся к 7 системно-органным классам: со стороны кожи и подкожной клетчатки; желудочно-кишечные нарушения; общие нарушения и реакции в месте введения; нарушения со стороны лабораторных и инструментальных данных; нарушения со стороны дыхательной, нервной и иммунной систем. В целом доля этих НР составила 84,1 % с вариацией значений в подгруппах от 78,2 % у моксифлоксацина до 91,9 % у ципрофлоксацина.</p></sec><sec><title>   Заключение</title><p>   Заключение. Для рационального использования фторхинолонов важна разработка комплекса мер по повышению общего уровня знаний врачей практического здравоохранения обо всём спектре НР, ассоциируемых сегодня с этой группой. Необходимо уже на ранних этапах выявлять не только распространённые, но и клинически значимые редкие НР фторхинолонов, и своевременно направлять полученную информацию в Росздравнадзор. Решение данных задач позволит в целом повысить безопасность антибактериальной терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Background</title><p>   Background. Fluoroquinolones continue to be widely used in the treatment of a wide range of infectious diseases, though new risks, including serious, have been identified in the process of their use. In order to maintain a favorable benefit-risk ratio, it is necessary to continue careful monitoring of the real-world safety profile of fluoroquinolones, which may allow, among other things, to develop new approaches to their rational use.</p></sec><sec><title>   Objective</title><p>   Objective. To study the structure of adverse reactions (ARs) associated with the use of fluoroquinolones based on a retrospective analysis of the national database of spontaneous reports.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. A retrospective analysis of spontaneous reports (SRs) received in the Russian database of ARs was carried out from 01.04.2019 to 28.02.2023. Inclusion criteria: SRs with ARs occurred on the territory of the Russian Federation, fluoroquinolone group drug indicated as a suspected drug (SD), identification of a high causal relationship "AR-SD". The Russianversion of MedDRA was used to code and systematize ARs.</p></sec><sec><title>   Results</title><p>   Results. The study included 1890 SRs, out of them 950 were for levofloxacin, 633 for ciprofloxacin, 205 for moxifloxacin, and 102 for other fluoroquinolones. 75.5 % of the received SRs contained information about serious ARs, more than 80 % of ARs developed at the inpatient stage of medical care, self-medication was detected in 2.9 % of cases. 40.8 % of SRs included data on young people; patient age ≥ 75 years was indicated in 7.7 % of SRs; in 75 cases (4.0 %) fluoroquinolones were used in children under 18 years of age. The analysis showed that the most frequently reported ARs were from next 7 system-organ classes (SOC): skin and subcutaneous tissue disorders; gastrointestinal disorders; general disorders and administration site conditions; investigations; respiratory, thoracic and mediastinal disorders, nervous system disorders and immune system disorders. In general, the proportion of these ADRs was 84.1 % with a variation in values in subgroups from 78.2 % for moxifloxacin to 91.9 % for ciprofloxacin.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. The development of a set of measures to improve the general level of knowledge of physicians about the entire spectrum of ARs associated with fluoroquinolones is essential for the rational use of this group. It is necessary to identify not only common, but also clinically significant rare ARs of fluoroquinolones at early stages, and promptly send the information to Roszdravnadzor.
