Антитабачная политика в России позволила сократить долю курильщиков до 30,7 %, однако около трети курильщиков не готовы бросить курить. Недооценка сложности полного отказа от курения является серьёзным барьером на пути улучшения здоровья населения. За последние годы накопилось достаточно данных о том, что электронные системы нагревания табака менее вредны для здоровья и являются достаточно эффективным средством отказа от курения.</p><p>   Целью данного исследования было моделирование потенциальных эффектов перехода всех курильщиков на использование электронных устройств нагревания табака.</p><p>   Эффектом считалось снижение числа смертельных случаев при использовании электронных систем нагревания табака (ЭСНТ) по сравнению со смертностью, связанной с курением.</p></sec><sec><title>   Методология</title><p>   Методология. При моделировании сравнивался «нулевой» сценарий с «альтернативным». «Нулевой» сценарий — это текущая ситуация с распространённостью курения, «альтернативный» сценарий предполагал, что все курильщики полностью перейдут на ЭСНТ, а снижение вреда для здоровья соответствует уровню снижения биомаркеров потенциального вредного воздействия продуктов сгорания табака на 16–49 % для пяти из 8 конечных точек на основе данных имеющихся исследований.</p></sec><sec><title>   Результаты</title><p>   Результаты. Переход мужчин от курения сигарет к ЭСНТ в возрасте 35–59 лет позволит спасти 39 102 жизни (19,1 % смертности, связанной с табакокурением) и 1,3 млн DALY-лет или 27,7 % потерь, которые сопровождают курение сигарет; у женщин в возрасте 35–59 лет потенциально предотвращается 3815 смертей (4,1 %) и экономится 822 тыс. DALY-лет или сокращение потерь на 15,7 %. Если учесть снижение токсичности от использования ЭСНТ на 49 % и пересчитать его на всю популяцию курильщиков, то переход к ЭСНТ может иметь существенный демографический эффект в плане снижения смертности, прежде всего в трудоспособных возрастах; эпидемиологический прирост показателей ВВП на душу населения может составить в 2022 году 1,5 трлн руб.</p></sec><sec><title>   Выводы</title><p>   Выводы. Продемонстрированное потенциальное снижение вреда для здоровья и существенный демографический эффект в плане снижения смертности при использовании электронных систем нагревания табака могут служить достаточно серьёзными доводами, которые необходимо учитывать при формировании регуляторной базы в отношении таких электронных устройств.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Relevance</title><p>   Relevance. The anti-smoking policy in Russia has reduced the proportion of smokers to 30.7 %; however, about a third of smokers are not ready to quit. Underestimating the complexity of completely quitting smoking is a serious barrier to improving the health of the population. In recent years, sufficient data has accumulated that tobacco heating systems (THS) are less harmful to health and are quite an effective means of quitting smoking.</p><p>   The objective of this study was to model the potential effects of all smokers switching to the use of THS.</p><p>   The effect was considered as a decrease in the number of fatalities when using alternative nicotine delivery systems ANDS, compared with the mortality associated with smoking.</p></sec><sec><title>   Methodology</title><p>   Methodology. The modeling compared the "zero" scenario with the "alternative" one. The "zero scenario" is the current situation with smoking prevalence, the "alternative scenario" assumed that all smokers completely switch to THS, and the reduction in health harm corresponds to the level of reduction in biomarkers of potential harmful effects of tobacco combustion products by 16-49 % for five of the 8 endpoints based on data from available studies.</p></sec><sec><title>   Results</title><p>   Results. The transition of men from cigarette smoking to THS at the age of 35-59 years will save 39,102 lives (19.1 % of tobacco-associated mortality) and 1.3 million DALY years or 27.7 % of the losses that accompany cigarette smoking. In women aged 35-59 years, 3,815 deaths (4.1 %) are potentially prevented and 822 thousand DALY years are saved or a reduction in losses by 15.7 %.
