Наравне с поиском оптимальных методов улучшения приверженности пациентов фармакотерапии и изучением факторов, оказывающих на неё влияние, не менее актуальной задачей остаётся выбор оптимального способа её оценки. В условиях активно протекающей во многих странах, в том числе в России, цифровизации систем здравоохранения, среди всех известных методов оценки приверженности наибольший интерес вызывает подход, основанный на анализе данных рецептурного отпуска. Наибольшее распространение среди вариантов данного подхода получил расчёт «доли обеспеченных дней» (ДОД). Однако, заимствование у зарубежных коллег опыта оценки приверженности с помощью ДОД оказалось затруднено наличием значимых проблем, связанных с недостаточной стандартизацией методики расчёта и интерпретации получаемых результатов. Целью данной работы являлась разработка стандартной методологии расчёта ДОД на базе данных рецептурного отпуска ЕМИАС для повышения соответствия требованиям, предъявляемым к качественным исследованиям приверженности фармакотерапии. Анализ литературы позволил выделить и описать следующие наиболее значимые элементы методологии расчёта ДОД: (1) критерии включения данных рецептурного отпуска; (2) продолжительность расчётного периода; (3) дата обращения за отпуском лекарственного препарата; (4) расчёт общего количества дней в расчётном периоде; (5) расчёт количества дней, обеспеченных рецептами; (6) интерпретация результатов расчёта ДОД; (7) оценка приверженности комплексной фармакотерапии. По результатам проведённого анализа, а также — опыта, проведённого ранее пилотного исследования, в заключении данной работы были представлены рекомендации, позволяющие проводить качественную оценку приверженности пациентов фармакотерапии с помощью расчёта ДОД.</p></abstract><trans-abstract xml:lang="en"><p>Adherence is one of the crucial determinants of high-quality and effective pharmacotherapy. Along with the search for optimal methods to improve adherence and with the study of factors influencing it, it remains an equally urgent task to choose the optimal method to evaluate it. In the context of the digitalization of healthcare systems that is actively taking place in many countries, including Russia, the approach based on the analysis of prescription data is of the greatest interest among all known methods of measuring adherence. The calculation of the «proportion of days covered» (PDC) has become the most widespread among the variants of this approach. However, the local adoption of foreign experience of assessing adherence by PDC turned out to be hampered by the presence of significant problems associated with insufficient standardization of the calculation methodology and interpreting the results obtained. The purpose of this work was to develop the standard PDC calculation method based on the EMIAS prescription claims data to increase compliance with the requirements for qualitative studies of medication adherence. The analysis of the literature made it possible to identify and describe the following most significant elements of PDC calculation methodology: (1) minimum fill criterion; (2) evaluation period; (3) method for identifying fill date; (4) calculation of the total number of days in the evaluation period; (5) calculation of the number of days covered with prescriptions; (6) interpretation of PDC calculation results; (7) assessment of multiple adherences.
