За последние четверть века в клинических исследованиях было показано, что назначение b-адреноблокаторов в тщательно подобранных дозах больным с ХСН, получающим стандартную терапию, уменьшает выраженность симптомов, повышает толерантность к физической нагрузке, улучшает функцию желудочков сердца (как систолическую, так и диастолическую), функциональный статус по классификации NYHA и снижает смертность. В ряде исследований, выполненных в последние годы, были получены чёткие доказательства того, что b-адреноблокаторы без внутренней симпатомиметической активности (ВСА) снижают смертность при ХСН. С другой стороны, и ретроспективный анализ данных нескольких рандомизированных исследований также выявил уменьшение смертности больных с ХСН, получавших b-адреноблокаторы. В связи с этим b-адреноблокаторы на сегодняшний день признаны одним из важнейших компонентов терапии ХСН.
